Current status of care for Cronic Kidney Disease (CKD) Patients

The Kidney Health for Life (KH4L) International Steering Group is conducting a multi-country research exercise on the current status of care for Chronic Kidney Disease (CKD) patients. KH4L is a new, international multi-stakeholder initiative whose goal is to work collaboratively with existing organisations to promote early detection and effective treatment of CKD. The ultimate goals of KH4L are to prevent CKD progression and improve the health and quality of life of people affected by CKD. Your response to this survey will facilitate a better understanding on how to provide quality healthcare that is easily accessible, and delivered in a timely and efficient manner with courtesy and respect to CKD patients.

All responses received will be kept completely confidential and you will not be identified in any report or publication arising from the survey as all the analyses will be aggregate and not based on individual responses. It should take about 15 minutes to complete.

Thank you in advance for your anticipated involvement.
